Getting Started

1. Create a form

Go to Forms in the sidebar and click New form. Give it a name, then add fields with Add field. For each field choose a type, mark it required if needed, and (for choices) list the options one per line. For a consent or waiver, choose the Consent type and write the statement the client must agree to.

2. Choose scope and frequency

  • Scope - Service to attach it to specific services, or Business to show it on every booking (e.g. a data-policy consent). You can have one active business form at a time.
  • Frequency - Once per customer so returning clients aren’t asked again, or Every booking for questions that change each visit.

3. Attach it to a service (service-scoped forms)

Open a service under Services, find the Intake form card, and select your form. Business-scoped forms apply automatically - no attaching needed.

4. Test it

Open your booking page and book the service. After the details step you’ll see the intake form as its own step.

Forms on WhatsApp Booking

If you use WhatsApp Booking (Max), the same forms appear as in-chat question pages before the booking is confirmed - no extra setup. One limit: WhatsApp shows at most 10 questions per form; longer forms are skipped there and stay web-only (the form builder tells you when a form crosses that line). For those clients you can send the form by email from their profile instead.
