Intake Forms

Pro Intake Forms are available on Pro and Max plans.

Intake forms let you collect the information you need from clients before their appointment - medical history, allergies, consent and liability waivers, or any custom question. You build a form once and it appears automatically during the booking flow.

How It Works

  1. Build a form - Add fields of any type (text, choices, yes/no, number, date, consent) under Forms.
  2. Decide where it applies - Attach it to specific services, or make it business-wide so it shows on any booking.
  3. Choose how often - Ask once per customer (e.g. a data-policy consent) or on every booking (e.g. “what to focus on today?”).
  4. Clients fill it at booking - The form appears as a step in the booking wizard; required fields must be completed.
  5. You read the answers - Responses appear on the client’s profile, where you can also add private notes.

Consent fields store the exact statement the client agreed to at the time of booking, so your record stays accurate even if you later edit the form.

Field Types

  • Short / long text - free-form answers
  • Single / multiple choice - predefined options
  • Yes / No - a simple toggle
  • Number and Date
  • Consent - an agree-to-statement checkbox for waivers and policies

Scope & Frequency

  • Scope: Service (only services you attach it to) or Business (any booking).
  • Frequency: Once per customer (returning clients are not asked again) or Every booking.
